Privacy Policy
Our Commitment to Privacy
Our web site Privacy Policy was developed as an extension of our
commitment to combine the highest-quality products and services with
the highest level of integrity in dealing with consumers, healthcare
professionals, investors, and other business partners using the Artes
Medical and ArteFill web sites. The Policy is designed to assist you in
understanding how we collect, use, and safeguard the information you
provide to us and to assist you in making informed decisions when using
our site and our products and services. This statement will be
continuously assessed against new technologies, business practices, and
the needs of our business.
What Personally Identifiable Information Do We Collect and When Do We Collect It?
When you visit our web site, you may provide us with 2 types of
information: information you knowingly choose to disclose that is
collected on an individual basis and web site-use information collected
on an aggregate basis as you and others browse our web site.
Information You Choose to Provide You
may register by filling out forms on our web site giving the following
information: first and last name, professional occupation, e-mail
address, street address and phone number. Health care professionals may
register by filling out forms on our web site giving all of the
information above plus fax number, board certifications currently held,
number of years in practice, the countries in which they are licensed,
and the states in which they are licensed (if licensed in the United
States). In addition, we will ask healthcare professionals to complete
a “Get Started Profile” that will provide us with information on their
use and experience with various aesthetic injectable products.
Providing the information described above is optional, however, it is
required for registration and the benefits that registration provides.
E-mail and Other Contacts With Us In
addition to providing the foregoing information, our web site gives
information about how you may contact us for further information about
our products. We provide our phone numbers, street address and e-mail
contacts. If you choose to correspond further with us, we may record
your telephone conversations or retain the content of your written
letters or e-mail messages together with your contact information and
our responses. We provide the same protections for these electronic
communications that we employ in the maintenance of information
received by mail and telephone. We have physical, electronic and
procedural security measures in place to protect the loss, misuse and
alteration of the information under our control. These security
measures include restricting access to your personally identifiable
information to our employees who need to know that information to
provide you with products or services, as well as a firewall to prevent
unauthorized access to our systems and maintenance of web servers in a
separate network isolated from the main network. Although we will
exercise reasonable care in providing secure transmission of
information between your computer and our servers, we cannot ensure or
warrant the security of any information transmitted to us over the
Internet, and we accept no liability for any unintentional disclosure.
We will protect the transmission of sensitive information, such as
credit card information, through industry standard, secure socket layer
encryption.
Web Site Use Information Cookies are a
feature of web browser software that allows web servers to recognize
the computer used to access a web site. Cookies are small pieces of
data that are stored by a user's web browser on the user's hard drive.
Cookies can remember what information a user accesses on one web page
to simplify subsequent interactions with that web site by the same user
or to use the information to streamline the user's transactions on
related web pages. This makes it easier for a user to move from web
page to web page and to complete commercial transactions over the
Internet. Cookies should make your online experience easier and more
personalized. Our web site uses cookies and web server logs to collect
information about how our web site is used. Information gathered
through cookies and web server logs may include the date and time of
visits, the pages viewed, time spent at our web site, and the web sites
visited just before and just after visiting our web site. This
information is collected on an aggregate basis. None of this
information is identifiable.
How Do We Use the Information That You Provide Us?
Broadly speaking, we use personal information for purposes of
administering our business activities, providing customer service, and
making available other products and services to our customers and
prospective customers.
Information You Voluntarily Provide If
you register with us or otherwise communicate with us, we may add your
contact information to our consumer, healthcare professional, or
investor relations database unless you indicate otherwise. When you
register with us, you will be given an opportunity to indicate whether
you wish to receive subsequent communications from us, such as
newsletters, updated product information and special promotions. If you
communicate with us using the contact information on our web site, we
may add you to our list for such subsequent communications. If we send
you information by electronic mail, each such e-mail will provide you
with an opportunity to “opt out” of receiving subsequent e-mail
messages. We will respect your choice whether to be on our lists for
receiving messages from us or not. If you do give us personal
information that we add to our database, the database is managed under
our traditional corporate standards designed to safeguard the security
and privacy of our customers' personal information.
We have
physical, electronic and procedural security measures in place to
protect the loss, misuse and alteration of the information under our
control. These security measures include restricting access to your
personally identifiable information to our employees who need to know
that information to provide you with products or services, as well as a
firewall to prevent unauthorized access to our systems and maintenance
of web servers in a separate network isolated from the main network.
Although we will exercise reasonable care in providing secure
transmission of information between your computer and our servers, we
cannot ensure or warrant the security of any information transmitted to
us over the internet, and we accept no liability for any unintentional
disclosure. Artes Medical will protect the transmission of sensitive
information, such as credit card information, through industry
standard, secure socket layer encryption.
Information We Collect From Cookies We
use web site browser software tools such as cookies and web server logs
to gather information about our web site users' browsing activities, in
order to constantly improve our web sites and better serve consumers,
healthcare professionals, and other interested parties. Cookies help us
collect important business and technical statistics. The information in
the cookies lets us trace the paths followed by users to our web site
as they move from one page to another. Web server logs allow us to
count how many people visit our web site and evaluate our web site’s
visitor capacity. We do not use these technologies to capture your
individual e-mail address or any identifying information about you,
although they do permit us to send focused online banner advertisements
or other such responses to you.
With Whom Do We Share Your Personally Identifiable Information?
As a general rule, we do not share information submitted by
consumers, healthcare professionals, or other interested parties with
any third party or company. However, we may disclose information you
provide if required to do so by law or if we have a good faith belief
that disclosure is necessary to (1) comply with the law or with legal
process served on us (2) protect and defend our rights or property or
(3) act in an emergency to protect someone’s safety. As we develop our
business, we may buy or sell assets, and, depending upon the
transaction, your information may be one of the transferred assets. In
the event that we are acquired by another company, your information may
be part of the assets transferred to the acquiring party.
How Do We Secure and Protect Your Personal Information?
We utilize encryption/security software to safeguard the
confidentiality of personal information we collect from unauthorized
access or disclosure and accidental loss, alteration, or destruction.
We use that software to encrypt the personal information stored in our
database and also to encrypt personal credit card information
transmitted to us through the forms on our web site. The information is
scrambled en route and decoded once it reaches our web site. Other
e-mail that you may send to us may not be secure unless we advise you
that security measures will be in place prior to your transmitting the
information. For that reason, we ask that you do not send confidential
information such as social security or account numbers to us through an
unsecured e-mail.
Periodically, our operations and business
practices are reviewed for compliance with corporate policies and
procedures governing the security, confidentiality, and quality of our
information. Our corporate values, ethical standards, policies and
practices are committed to the protection of customer information. In
general, our business practices limit employee access to confidential
information, and limit the use and disclosure of such information to
authorized persons, processes, and transactions.
What About Other Web Sites Linked to Our Web Site?
We may provide links to our strategic partners and to trained
physicians who prescribe our products. We are not responsible for the
practices employed by other companies or the physicians referenced on
our web site. Please remember that when you use a link to go from our
web sites to another web site, our Privacy Policy is no longer in
effect. Your browsing and interaction on any other web site, including
web sites which have a link on our web site, are subject to that web
site’s own rules and policies. Please read over those rules and
policies before proceeding.
